Sample (SP5) form

Entitlements Declaration Form (SP11) form, submit it with your SPS 2009 application.

This form should be used to tell RPA that you wish to assign any payment due to you from SPS 2009 (incorporating Area Payment for Nuts, Aid for Energy Crops, Protein Crop Premium and Hill Farm Allowance 2010 (HFA)) to a third (assignee) party.

If you have already sent us a Payment Redirection Form (CReg08) to register details of a third party to whom you wish payments to be re-directed, and you would like us to make your SPS 2009 payment to that person, please use complete the SP13 form below. Completion of the SP13 form will give us the authority to make payment to that account.

If you have not previously registered a third party to whom you wish payments to be re-directed, you will need to complete a CReg08 form. The CReg08 registers the full details of the third party and the SP13 gives us the authority to pay your SPS 2009 payment to that account.

The SP13 form applies to the 2009 scheme only. (Except HFA 2010.)
