SPS 2006 application form - additional guidance
In response to a number of requests from customers for further guidance, we have provided a sample of the 2006 application form with annotated guidance on how to complete specific boxes. This should prove useful if you have not yet completed your application form or wish to check it before submitting it. We have also had a number of queries with regard to information on entitlement statements. We have recently updated our FAQ’s on this website to provide further information on this subject, particularly to cover the case where you think the information provided on the form is wrong.
RPA recognises that delays caused in the processing of 2005 SPS claims may lead to potentially inaccurate SPS claims being filed in 2006. However, to the extent that any such inaccuracies are a direct result of our actions, we will ensure that any affected 2006 SPS claims will not be prejudiced. The contents of the signed declaration in the SP5 continues to apply in every case. Please also note that you must still ensure that your completed SP5 is received by us by 15 May in order to avoid possible late claim reductions, or even rejection of your claim if it is received after 9 June.
To print out a copy:
1. Hold your mouse over the icon.
2. Using the right mouse button click once.
3. Select the option 'Open Link in New Window'.
4. Print as normal.
