Agent Authorisation Form SP9


If you are signing the SPS application or RLE1 as an agent or partner you must be authorised to make the application.

If an SP9 has previously been submitted, the end date has not expired and no changes have occurred, you do not need to submit a new one.

If there have been changes, or if you have not been authorised via an SP9 or CREG01, you must submit an SP9 (this can be submitted with your with your RLE1 form or SPS application).

Please note this SP9 form contains the address for Newcastle. We will accept your completed SP9 form whether it is returned to Newcastle or when it is returned to Sheffield as a supporting document with your SPS 2010 application form.

It is recommended that you check your printer settings before you print the blank form, in case the form is not printed correctly, i.e. not all character boxes and fields may be printed because they are outside normal printer boundaries.
 
To print out a copy:
 
1. Hold your mouse over the icon.
 
2. Using the 'right' mouse button click once.
 
3. Select the option 'Open Link in New Window'.
 
4. Print as normal.
